COPD Task and Finish Group
In Pennine Lancashire (the area that incorporates East Lancashire Hospitals NHS Trust, and Bwd CCG and East Lancs CCG) the number of attendances to A&E, and admissions to hospital are higher than we would like. Most A&E attendances and admissions are not necessary and can be prevented through self care and the right support from the NHS and other agencies.

The task and finish group will oversee the development and implementation of an integrated service model for COPD. This model will specifically include a focus on admission avoidance and early supported discharge. Here is the groups' Terms of Reference (the aims and objectives):

One innovation that the group are considering is the British Lung Foundation's COPD Patient Passport. Its a way of assessing whether you are receiving the right range and level of care for COPD.
